Work permit request forms are available in the Career Center.  They are hanging up just inside the door.  You can also download them here.  If you are under 18 and working, you must obtain a work permit unless you have graduated from high school, passed the California High School Proficiency Exam or the California GED.  Work permits are granted on the basis of age, grades and attendance, and your employer must provide you with Worker's Compensation Insurance.  Regulations for 14-15 year olds, click here.  Regulations for 16-17 year-olds, click here.
Employer Information Confused about the rules when you hired that high school student?  If they are under 18 and still in school, THEY REQUIRE A WORK PERMIT.  THEY ALSO MUST BE COVERED BY WORKERS COMPENSATION INSURANCE.  The Federal Fair Labor Standards Act can be found here, and State regulations can be found here.
